Here is what you do.

#1....Print out a copy of the above PDF repair form. (Make copies if you are sending in multiple items).

#2....Please remove and keep any detachable cables, adapters, power cords, etc. 
(If your printer uses a external Power Supply, send it with the printer)
  
 #3....Please include as much information on the repair form as you can.  Tape a copy of the form to top of printer.
  
#4....Please pack the Printer Securely in a box that will protect it from any shipping damage.
(You may want to add Shipping Insurance for your protection, we a not responsible for damage).

Your printer will normally be repaired within 5-7 working days.

It will be Shipped back UPS Ground for a Flat Fee of $25.00 to $55.00 each Unit, depending on your location.
 (If you are outside the Lower 48 States, please call for a Quote).

If in California there is a Tax of 9.25% on Parts Only.
